Cheryl Polkowski, MD, is a board-certified family medicine physician at UT Health East Texas Physicians in Jacksonville and Rusk who embraces her patients as family. Originally from upstate New York, Dr. Polkowski moved to central Texas in 2008 and built a successful practice. She recently relocated to East Texas to pursue her interest in rural health with the goal of caring for extended families for years to come. Dr. Polkowski provides a wide range of medical services from newborn and child healthcare to managing complex, chronic illnesses in adults and the elderly.
Dr. Polkowski also has a passion for hospice care, bringing patients and their families comfort and counsel through end of life decisions.

Dr. Polowski has been my doctor for 12+ years. She and her staff will bend over backwards to do the very best for her patients. Dr. Polkowski is probably the best physician that I have ever had. I am a former patient of hers with the S & W Health Plan . I didn't renew my Insurance with S & W because I'm going to purchase new Insurance in hopes of going back to Dr. Polkowski. She genuinely cares about her patients.
